About Children’s Hospital LA
Children’s Hospital Los Angeles on Sunset Boulevard is one of the country’s leading children’s hospitals. Here we usually notarize for parents and guardians, not the patients themselves.
We are an independent mobile notary service. We are not affiliated with, endorsed by or acting for Children’s Hospital Los Angeles. We come at the request of patients, inmates and their families.
Before we arrive
- Tell us the unit and room, and whether the patient is in the ICU or on isolation precautions
- Ask the nurse when the patient is usually most alert, and book for then
- Have the patient’s ID at the bedside: a California driver’s license or ID issued within five years, a passport, or another acceptable ID. A hospital wristband isn’t ID
- If there’s no acceptable ID, arrange two credible witnesses who know the patient and aren’t named in the document (the agent named in a power of attorney can’t be one)
- Have the documents printed and complete, or send them to us to print
What patients and families usually sign
- Durable powers of attorney, so family can handle bills and accounts
- Advance health care directives. If witnessed instead of notarized, hospital staff can’t be witnesses
- Trust and property documents
- Insurance, bank and employer forms that ask for a notary
If the patient can’t write, they can sign by mark with two witnesses. For powers of attorney and deeds, the journal thumbprint can come from any available finger.
For parents and guardians
Parents often need documents while a child is in the hospital: consent letters authorizing a relative to make decisions, powers of attorney for their own affairs, travel consent letters and passport consent forms for treatment abroad, and employer or insurance forms. Each signer needs acceptable ID.
Common questions
Can a notary come to Children’s Hospital LA tonight?
Often, yes. We’re available 24/7, including nights, weekends and holidays. Call (424) 484-9268 for the fastest response.
What if the patient is sedated or confused?
We can only notarize if, at the time of signing, the patient can communicate with us, understands the document and wants to sign. If that isn’t clear, we can’t proceed; it may be possible to come back at a better time.
Can hospital staff be witnesses?
Many hospitals don’t allow it, and health care providers and their employees can’t witness an advance health care directive. We can arrange witnesses for an additional fee.
