Quick answer
We’re the notary hospital social workers, case managers and care facility staff call for patients and families: powers of attorney, advance health care directives, trust and property documents, signed at the bedside at any hour. We also notarize documents for healthcare businesses, such as provider and credentialing documents, affidavits and corporate paperwork. Every notary is NNA certified and insured with at least $100,000 in E&O coverage.

For patients and families
- Durable powers of attorney and advance health care directives
- Trust, deed and banking documents
- Patients without current ID, identified by credible witnesses
- Patients who can’t write, using signature by mark
If the patient is in a skilled nursing facility, an advance health care directive also needs a patient advocate or ombudsman as a witness (Probate Code § 4675). We’ll help coordinate timing so everyone is present.
What we need from staff
- The patient’s location, and the best time of day for alertness
- Whether the patient has acceptable ID, or who can serve as credible witnesses
- Visitor and check-in procedures
- A heads-up if staff can’t serve as witnesses under facility policy, so we can arrange witnesses
We can’t notarize if the patient can’t communicate with us, appears not to understand the document, or seems pressured. We’ll say so respectfully and leave the next steps to the family and their attorney.
For healthcare businesses
Clinics, medical groups and home health agencies use us for provider and credentialing documents, affidavits, corporate documents and anything going abroad that needs an apostille. We come to your office.
Common questions
How quickly can you get to a hospital?
Often within a few hours, sometimes within the hour on the Westside. We’re available 24/7.
Can hospital staff act as witnesses?
Facility policies often prohibit it, and health care providers and their employees can’t witness an advance health care directive. We can arrange witnesses for an additional fee.
