About Northridge Hospital

Northridge Hospital Medical Center on Roscoe Boulevard serves Northridge, Chatsworth, Porter Ranch and the surrounding Valley.

We are an independent mobile notary service. We are not affiliated with, endorsed by or acting for Northridge Hospital Medical Center. We come at the request of patients, inmates and their families.

Before we arrive

  • Tell us the unit and room, and whether the patient is in the ICU or on isolation precautions
  • Ask the nurse when the patient is usually most alert, and book for then
  • Have the patient’s ID at the bedside: a California driver’s license or ID issued within five years, a passport, or another acceptable ID. A hospital wristband isn’t ID
  • If there’s no acceptable ID, arrange two credible witnesses who know the patient and aren’t named in the document (the agent named in a power of attorney can’t be one)
  • Have the documents printed and complete, or send them to us to print

What patients and families usually sign

If the patient can’t write, they can sign by mark with two witnesses. For powers of attorney and deeds, the journal thumbprint can come from any available finger.

Common questions

Can a notary come to Northridge Hospital tonight?

Often, yes. We’re available 24/7, including nights, weekends and holidays. Call (424) 484-9268 for the fastest response.

What if the patient is sedated or confused?

We can only notarize if, at the time of signing, the patient can communicate with us, understands the document and wants to sign. If that isn’t clear, we can’t proceed; it may be possible to come back at a better time.

Can hospital staff be witnesses?

Many hospitals don’t allow it, and health care providers and their employees can’t witness an advance health care directive. We can arrange witnesses for an additional fee.
