No. Choosing a legal document, filling it in for you or explaining what it means is the practice of law, and our notaries aren’t attorneys. Ask an attorney, or whoever asked you for the document, which form they accept and what goes in it.
Will the person or agency I’m giving it to accept this template?
We can’t promise that. Many airlines, banks, hospitals, courts and agencies have their own forms or requirements. Check with them before you buy.
Are there free versions?
Some, yes. The text of the California advance health care directive is published free in Probate Code § 4701, and the DMV has its own free bill of sale (REG 135). Our templates are typeset, fillable versions with instructions for signing and notarization.
Can I get a refund?

Do I sign the form before my appointment?
