What to have ready
- The patient’s name, the medical center, the unit (ER, ICU or floor) and the room or bed, and whether they might be moved
- The patient’s ID: a California driver’s license or ID card, a passport, or another acceptable ID. A hospital wristband or Kaiser member card isn’t ID
- If there’s no acceptable ID, two credible witnesses who know the patient and aren’t named in the document
- The documents printed with every blank filled in, or emailed to us to print
- Witnesses, if the document needs them. Hospital staff usually can’t act as witnesses, but we can bring them for an additional fee

Common questions
Can a notary come to the Kaiser emergency room?
Yes. We notarize in the emergency department, including for patients in hallway beds or waiting to be admitted. Call us rather than booking online so we can move quickly.
Does Kaiser have a notary on staff?
Not one families can count on. Most book a mobile notary so the signing happens when the patient is alert and ready, day or night.
Is a Kaiser member card acceptable ID?
No. The patient needs an acceptable government ID, or two credible witnesses who personally know them.
How fast can you get to a Kaiser hospital?
Often within a few hours, and sometimes within the hour at Kaiser West LA. We’re available 24/7, including nights and holidays.
