Nursing Home Notary: We Come to the Bedside, 24/7

A parent recovering in skilled nursing or rehab who needs a power of attorney or health care directive signed? We come to the bedside, any day, any hour.

Home Notary Public Nursing Home Notary

Quick answer

Yes. We come to nursing homes, skilled nursing facilities and rehab centers across Los Angeles County, 24/7, to notarize powers of attorney, trust and property documents and other paperwork at the bedside. One rule is special here: an advance health care directive signed by a skilled nursing resident isn’t effective unless a patient advocate or ombudsman also signs it as a witness (Probate Code § 4675). Arrange the ombudsman before we arrive.

The ombudsman rule for health care directives

For a resident of a skilled nursing facility, an advance health care directive isn’t effective under Probate Code § 4675 until a patient advocate or ombudsman has signed it as a witness, and that holds even when it’s notarized. The facility’s social services staff can tell you how to reach the long-term care ombudsman. Schedule them for the same time as the notary so the resident signs once.

Powers of attorney, trusts and deeds don’t have this rule. See advance health care directives.

What residents and families usually sign

Before we arrive

  • Tell the front desk or nurse a notary is coming, and ask when the resident is usually most alert
  • The resident’s ID: a California driver’s license or ID card or a passport, current or issued within five years. Expired? Two credible witnesses who know the resident can identify them instead
  • The documents printed with every blank filled in, or emailed to us to print
  • Any witnesses the document needs. We can bring witnesses for an additional fee

Care facility visits carry an added travel fee; see our Policies. Call (424) 484-9268 for the total.

Common questions

Is there a nursing home notary near me?

Yes. We come to nursing homes, skilled nursing facilities and rehab centers anywhere in Los Angeles County, 24/7 by appointment, often the same day. Call (424) 484-9268 for the fastest booking.

Does the ombudsman have to be there if the directive is notarized?

Yes, for a skilled nursing resident. The ombudsman or patient advocate signs as a witness even when the directive is notarized.

Can nursing home staff be witnesses?

Usually not for health care directives, and many facilities don’t allow it at all. Bring your own witnesses or ask us to bring them.
